About the Clinic Resources

We have developed the following resources to help guide the implementation of the Saferteens program in primary care settings: 

• Seamless Referral Guide tools that can help clinicians use a patient-centered, Motivational Interviewing-inspired approach to seamlessly refer a patient to the Saferteens interventionist.

• Patient Card that your clinic can give to patients to tell them more about the Saferteens program.

• Paper versions of our 3-item Saferteens screening survey and instructions for interpreting a patient's screening results.
