Screening Survey

We have the following printer-friendly versions of the screening survey available for download: 

• A screener for use in primary care settings, which inquires about aggressive behavior in the past year

• A screener for use in emergency department settings, which inquires about aggressive behavior in the past 3 months

For both survey versions, a patient screens positive to receive the Saferteens intervention if he/she indicates participating in at least 1 of the listed behaviors one or more times during the timeframe asked about.
