About the Patient Resources

We have developed resource brochures that can be adapted for your clinical setting. In this section you can find:

• A template that you can download and modify so you can create your own community resource brochure that is applicable to your youth population and clinical setting.
• An example resource brochure that we use in our Flint, MI community.
• Our national resource brochure, which contains links to resources throughout the country.

All of the resource brochures can be downloaded and printed.
